  • Camp IWANNAGO Parent Guardian Agreement

    • Children must be between 5 and 12 years old and be able to use the restroom unassisted.
    • All contract requests are subject to approval based on available space.  
    • An authorized adult must walk my children up to the Camp IWANNAGO entrance and sign them in in the morning. An authorized adult must also walk up to the entrance to sign them out.
      • Anyone picking my child up from FACTS, including myself, is required to show a photo ID to any team member upon request.
      • Only adults authorized to pick up my child will be allowed to do so. The primary account holder can edit the authorized pick-up list by logging in to their Eleyo account.
    • No external adult will be allowed to enter the school building unless invited by a Camp IWANNAGO team member. This includes parents and all authorized emergency contacts.
    • My child’s behavior can result in suspension or expulsion. Use of this measure is at the discretion of the site Enrichment Specialist and FACTS Coordinator. Where appropriate, other age-appropriate behavior management techniques will be used prior to suspension or expulsion. FACTS Behavior Policy is available by going to the FACTS website
    • If my child has not been picked up within 30 minutes after the facility closes, law enforcement officials will be called to pick up my child.
    • I agree not to hold any person(s) or organization connected with FACTS and Flagstaff Unified School District, responsible for injuries or illness sustained by my child in the normal course of play. I understand that said organization does not provide medical insurance coverage for my child. I am responsible for the medical bills if my child is injured or becomes ill.
    • Camp provides a nutritious snack before 4:00 pm daily. Snack menus are posted on the FACTS website.
      • Students are encouraged, but never required to eat snacks.
      • Withholding of snacks is never used as a punishment.
      • If my student has special dietary needs, I am required to send a snack with my child when the scheduled snack does not meet their needs.
    • On days that I am registered for both the AM/PM and Full day sessions, FACTS staff have my permission to sign my child out of the AM section and into the PM Section. 
    • FACTS team members may dispense oral medication, Epi-Pens and inhalers only with the written permission form completed. It can be downloaded from the FACTS Registration Information Page.
    • I must inform a Camp IWANNAGO team member of any changes to the information provided on the Emergency Card.
    • Field Trips
      • All field trips require a separate permission form to be completed by the parent or guardian.
      • My child must be dropped off and picked up at the program site, not at any field trip location.
      • I must have my child signed in to Camp IWANNAGO by the time listed on the field trip permission form in order for my child to attend.
    • Camp IWANNAGO may not be able to accommodate all dates requested, and it is the parent/guardians' responsibility to review their Child's schedule after they receive an approval notification.
    • Changes to a camper schedule can be made by Wednesday of the week prior to the date of the schedule change.  
      • Additional dates are subject to approval. 
      • You can request a drop-in day after the Wednesday prior to the request changes.  Drop-in days are an additional charge.
